Why Teeth Need to Be Extracted

Teeth with deep cracks or fractures that extend below the gum line often cannot be saved and must be removed. Advanced periodontal disease can weaken the bone and ligaments supporting the tooth beyond repair. Sometimes a tooth becomes impacted, as with wisdom teeth, or it may be positioned in a way that interferes with other teeth or orthodontic treatment.

Simple Extractions versus Surgical Extractions

Surgical extractions are more complex and are needed for teeth that are impacted, have curved or multiple roots, or are deeply embedded in bone. This includes most wisdom teeth. During a surgical extraction, Dr. Rahmani may need to make a small incision in the gum tissue, remove a small amount of bone to access the tooth, or section the tooth into multiple pieces for easier removal. Surgical extractions require more advanced anesthesia and sedation options, and the healing process takes longer. However, Dr. Rahmani's experience and skill make even complex surgical extractions safe and efficient.

Wisdom Teeth Extraction

Wisdom teeth, the third molars that typically emerge in late adolescence or early adulthood, often cause problems and may need to be removed. Many people don't have enough space in their jaw to accommodate wisdom teeth, causing them to become impacted or only partially erupt through the gum. Partially erupted wisdom teeth are difficult to clean and are prone to decay and gum disease. Impacted wisdom teeth can damage adjacent teeth and cause infection.

The Extraction Procedure

Before your extraction, we'll provide thorough instructions on how to prepare. You may be asked to avoid eating or drinking for a certain period beforehand, and we'll discuss any medications you're taking. Bring a list of allergies and medical conditions, and arrange for someone to drive you home if you're receiving IV sedation.

The procedure begins with local anesthesia to numb the area. If you're anxious or undergoing surgical extraction, we offer nitrous oxide (laughing gas), oral sedation, or IV sedation to help you relax. Once you're comfortable, Dr. Rahmani carefully examines the tooth and surrounding bone. For simple extractions, she'll use instruments to gradually loosen the tooth until it can be gently removed. For surgical extractions, she may make a small incision, remove bone as needed, section the tooth if necessary, and then extract each piece carefully.

Throughout the procedure, we monitor your comfort and vitals. Once the tooth is removed, we may place bone graft material or a growth factor to promote healing and prepare the site for future tooth replacement. We'll provide specific post-operative instructions and will likely see you for a follow-up appointment to monitor healing and ensure proper recovery.

What to Expect After Extraction

Following extraction, some swelling, bruising, and discomfort are normal. We'll provide detailed aftercare instructions including how to manage pain, control bleeding, and care for the extraction site. Most people can return to light activities within a few days, though complete bone healing takes several months. During the first 24 hours, avoid rinsing, spitting, or drinking through straws, as these actions can dislodge the blood clot and cause a painful condition called dry socket.

You'll receive specific information about pain management, dietary restrictions, oral hygiene during healing, and signs of complications that would require immediate attention. Follow these instructions carefully to ensure proper healing. Most people feel dramatically better after a few days and can return to normal activities within a week, though complete bone healing continues for several months.

Tooth Replacement Options

Once a tooth is extracted, we strongly recommend replacing it. Missing teeth can cause serious problems. Adjacent teeth shift toward the gap, changing your bite and making it difficult to clean between teeth. The tooth above or below the gap may erupt excessively. Over time, bone resorbs where teeth are missing, changing your facial structure and making you look older. Missing teeth affect your ability to chew properly and can impact your nutrition and digestive health.

There are several excellent options for replacing missing teeth, and the best choice depends on your specific situation, budget, and preferences. A dental implant is a titanium post surgically placed in the jawbone that serves as an artificial tooth root. Once integrated with the bone, a crown is attached to the implant to restore appearance and function. Implants are the most tooth-like replacement option, they preserve bone, and they can last a lifetime with proper care. However, they require surgery and a healing period of several months.

A fixed bridge uses adjacent teeth as anchors to hold a prosthetic tooth in place. The adjacent teeth must be prepared, which involves removing some natural tooth structure. A bridge is less expensive than an implant and doesn't require surgery, but it does compromise the adjacent teeth and doesn't prevent bone loss. A removable partial denture is another option, particularly if multiple teeth are missing. It's the least expensive option and requires no surgery, but it must be removed for cleaning and may affect your ability to taste food and eat comfortably.
